New Patient Information:
If you are a new patient, please bring your insurance card and the completed new patient forms below with you to your appointment to ensure timely processing. If you have been under the care of another physician, please also bring your records or have them sent prior to your appointment.
Please print and complete the five new patient forms below prior to your scheduled doctor visit:

- Notice of privacy practices
- Authorization to use and disclose protected health information
- Electronic prescribing notice
- Patient medication log

Community Medical Associates is a physician and certified mid-level group of more than forty providers.
CMA One is located at 2142 Sunset Drive and includes family practice, gastroenterology and neurology.
CMA Two is located at 2141 Hamilton Way and includes pediatrics.
CMA Three is located at 3350 Executive Drive and includes internal medicine and the Heart & Vascular Center, which houses interventional cardiology, diagnostic cardiology, and cardiovascular and thoracic surgery physicians.
OB/GYN providers are located in the Williams Family Center, right next to our birthing center at San Angelo Community Medical Center.
